For Colorado entities, mail the form 8822B to:

To report any ownership change, use your organization’s business letterhead to provide the notification. A corporate officer, LLC manager or partner must sign this notification. The fee is applied to make this change with Colorado Department of Revenue.

Colorado Secretary of State:

As of July 1, 2000, the Colorado Revised Statutes were changed so that entities are no longer required to file officers’, owners’, or directors’ names and addresses with the Secretary of State’s office. This information should be managed through your internal records.
